Section 5. PIAA Re-Certification by Parent/Guardian

To participate in a winter or spring sport requires the completion of the recertification form by the parent.

This is Section 5 of the PIAA Comprehensive Physical Form. The form can be accessed by going to www.piaa.org. Go to the parent section and then forms.

The form, when completed, should be turned into the coach or athletic office.

Section 6. PIAA Comprehensive Pre-Participation Re-Evaluation and Re-Certification By Authorized Medical Examiner.

If there has been lost injury time that occurred during the previous sports season, the parents should indicate this on the form. If you check yes, then you need to explain yes.

This may require prior to participation completion of Section 6 by a medical examiner. This is the PIAA Comprehensive Pre-Participation Physical Re-Evaluation and Re-Certification by Authorized Medical Examiner.
